Diabetes Program Services
Individual Counseling:
All Diabetes patients receive a one hour initial individual comprehensive assessment and plan development session. Individual sessions available for all educational needs. Individual sessions are utilized for yearly follow-up or tune-up sessions.
Group Classes:
Available for newly diagnosed type 2 diabetes patients or type 2 diabetes patients who have been previously diagnosed but have never received comprehensive education in the past. Held every other month at both sites in four, 2 hour sessions. Family or friends are welcome to attend.
Through our counseling, we provide:
- Initial education or follow-up/tune up education
- Comprehensive Diabetes Assessment: Incorporates the individual person, all health issues, medications, current lifestyle, and health beliefs. Works in conjunction with your physician and your medical treatment plan
- Communication with your doctor
- Patient and educator-developed education plan
- Patient and educator-developed goal plan
- Behavior change strategies and planning
